GHSA Physical Form

Any student wanting to participate in a sport must have a physical on file before beginning practice. Physicals may be given to each coach.

GHSA physical form that must be used for all physicals, instructions for creating an dragonfly account, and directions on how to upload physicals to Dragonfly. All students that want to participate in any activity at Lakeside must have a current athletic physical uploaded on Dragonfly.
